About the role
- Help elevate patient care with 25-50% travel within Southwest Michigan
- Work standard business hours - 8:00AM-5:00PM, no night, weekend or holiday shifts
- Develop proactive plans of care for members to enhance outcomes and overall wellness
- Conduct evaluations of member's needs and benefits using clinical tools
- Collaborate with members, providers, and community organizations
- Use motivational interviewing skills for maximum member engagement
- Present cases at interdisciplinary case conferences
Requirements
- Minimum 3+ years clinical practical experience as an RN
- Active and unrestricted Michigan RN licensure
- Willingness to travel up to 25%-50% of the time in specific counties
- Reliable transportation required
- 2+ years of experience in case management, discharge planning and/or home health care coordination experience preferred
- Behavioral health experience preferred
- Bilingual in Spanish or Arabic preferred
- Certified Case Manager preferred
- Additional national professional certification (CRC, CDMS, CRRN, COHN) preferred
- Associate's Degree required
- Bachelor's degree preferred
