About the role
- Full-time telework Health Coach Consultant role providing assessment, planning, implementation and evaluation to engage, educate, and influence members' health decisions.
- Conduct comprehensive evaluations of members' needs and benefit plan eligibility and utilize assessment techniques to determine health literacy, technology capabilities, and readiness to change.
- Apply clinical guidelines, health/wellness management plans, and policies to ensure appropriate utilization of services and quality of care; escalate quality of care issues when needed.
- Use motivational interviewing and coaching to empower members, develop and monitor care plans, and document care in compliance with regulatory and accreditation guidelines.
Requirements
- Active and unrestricted compact RN licensure with ability to apply for and obtain additional licenses required per account needs.
- Must reside in a compact state and hold multistate nursing license.
- 3+ years of clinical experience.
- Must possess or be willing and able to obtain high speed internet access and be hardwired to internet.
- Associate or Bachelor’s degree required in Nursing.
- Preferred: Previous coaching experience, Managed care experience, Experience with Microsoft Word, Outlook, Excel, and comfortable using various computer programs.
