About the role

  • - Review and organize patient information remotely to support care coordination and provider follow-ups.
  • - Assist with patient assessments by gathering relevant details through chart review, intake information, and care coordination tasks.
  • - Update patient records, care plans, and documentation in coordination with healthcare providers.
  • - Review and manage medication lists and support medication refill coordination by routing requests to providers or pharmacies.

Requirements

  • **Skills**
  • - Conversational Spanish proficiency (spoken) required; ability to understand and respond in common patient-care scenarios.
  • - High attention to detail when handling healthcare documentation and patient records.
  • - Clear verbal and written communication skills with patients, families, and healthcare teams.
  • - Proficiency with telehealth platforms and virtual communication tools.
  • - Strong organizational and time-management skills.
  • - Working knowledge of HIPAA and patient confidentiality requirements.
  • **Experience**
  • - Experience in a remote, virtual, or healthcare support role preferred.
  • - Experience using Electronic Health Record (EHR) systems (e.g., Epic, Cerner, or similar).
  • - Experience supporting medication coordination processes and patient follow-ups is a plus.
  • - Familiarity with scheduling tools and secure messaging platforms is an advantage.
