About the role

  • Provide programmatic oversight for all center-based services and participant supports
  • Ensure programming aligns with the Concordance Model, clinical best practices, and trauma-informed principles
  • Support staff in managing complex participant needs, including escalation and case staffing
  • Maintain compliance with CARF, HIPAA, state regulations, and internal protocols
  • Lead implementation of new programming or interventions in collaboration with senior leadership
  • Directly supervise Care Team Members, providing coaching, support, and accountability
  • Hold weekly team meetings to coordinate care, align practices, and share updates
  • Conduct monthly 1:1 supervision sessions with direct reports
  • Deliver clinical supervision as appropriate based on licensure
  • Identify training needs and support professional development
  • Foster a culture of collaboration, empathy, high performance, and trauma-informed care
  • Audit participant records and sessions to ensure accuracy, completeness, and fidelity
  • Oversee documentation standards, ensuring timely and compliant entries
  • Monitor caseloads, service delivery patterns, and team productivity
  • Track and report key program metrics, participant outcomes, and progress trends
  • Develop and implement quality improvement strategies based on data insights
  • Ensure a consistent, healing, dignified experience for participants across all services
  • Support creative, policy-aligned problem solving to reduce barriers to engagement
  • Address participant escalations and collaborate with staff to resolve issues
  • Advocate for participant needs across multidisciplinary teams
  • Partner with Concordance’s senior leadership to implement strategies, enhance programming, and improve team member experience

Requirements

  • Master's degree in counseling, social work, behavioral health, or related field
  • LCSW, LPC, or related license
  • 5-7 years of experience in social work, behavioral health, case management, counseling, or a related field
  • 3-5 years of experience of people leadership
  • Experience supervising multidisciplinary teams preferred
  • Strong understanding of trauma-informed care, behavioral health, and evidence-based practices
  • Experience working with justice-involved adults or other vulnerable populations is strongly preferred.
