About the role
- Deliver 'top of license' care through direct patient visits across home settings in the community
- Guide and inspire a team of field-based advanced practice clinicians to elevate clinical quality, performance, and outcomes
- Use data and KPIs as tools to achieve real, measurable change
- Build trusted relationships with key clinical and community partners, including SNFs, hospitals, and home health providers
- Support onboarding, mentorship, and ongoing performance development for APP team members
- Support value-based care initiatives executed by cross-functional teams
Requirements
- Certified Nurse Practitioner or Physician Assistant with active licensure and DEA credentials in New York
- 3+ years of clinical experience with aging or medically complex populations
- Proven leadership capabilities and experience managing clinical teams
- Public health, business administration, and/or health administration studies preferred
- Previous experience with managed care and/or value-based healthcare delivery preferred
- Passion for health outcomes, innovation, and delivering whole-person care
- Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation
- Proof of liability and property damage insurance on vehicle used is required
- DRIVER REQUIREMENTS: Licensed for a minimum of 3-5 years
- No vehicle-related suspensions/reinstatements, DUI, reckless driving, or felony convictions within the last 7 years
- Multiple violations and accidents within the last 5 years would be subject to review.
