About the role
- Assess patient and family needs, provide nursing care in the home environment.
- Collaborate with Interdisciplinary Group (IDG) to develop and direct individualized care plans.
- Document and manage patient care issues in collaboration with the healthcare team.
- Participate in on-call duties as defined by the organization.
- Maintain productivity standards and adapt to the dynamic needs of patient care.
Requirements
- Associate or Bachelor's degree from an accredited School of Nursing.
- Current, unencumbered (State of Employment) License as a Registered Nurse.
- National Provider BLS - American Heart Association certification required within 30 days of hire.
- 1 year experience practicing as a registered nurse preferred. Experience as a Hospice CNA or Hospice LPN will be considered.
