About the role
- Deliver personalized care to patients in their homes
- Assess, treat, and consult to help patients regain functional independence

Requirements
- Bachelor's degree in Physical Therapy or higher
- Current, unencumbered State License as a Physical Therapist
- National Provider BLS from American Heart Association
- 1 year of Physical Therapy experience with adults in a home health care or rehabilitation setting preferred
