Match service locations’ capabilities to patient needs and facilitate patient flow from acute care providers into Stonerise to meet/exceed census and goals
Communicate with the Interdisciplinary team daily to ascertain information about open beds, scheduled discharges, scheduled admissions, hospitalized patients, clinical capabilities, and center events
Manage the electronic referral system for assigned centers and develop professional relationships with case managers and discharge planners, hospital employees and management, patients, and families
Complete clinical reviews via Allscripts and/or onsite evaluation as needed or requested, and coordinate a seamless transition to assigned centers
Remain visible as assigned hospital and be available as a resource to promote Stonerise in a positive manner
Abide by all HIPPA laws and observe confidentiality policy
Present the benefits of long and short-term skilled nursing at Stonerise to patients, families, hospital employees, etc.
Requirements
Two (2) years of experience in sales and marketing
Current and valid driver’s license with available transportation and ability to drive
