About the role
- Facilitate admissions from assigned hospitals, clinics, and MD offices into Stonerise agencies
- Match service locations’ capabilities to patient needs and facilitate patient flow from acute care providers into Stonerise to meet/exceed census and goals
- Communicate with the Interdisciplinary team daily to ascertain information about open beds, scheduled discharges, scheduled admissions, hospitalized patients, clinical capabilities, and center events
- Manage the electronic referral system for assigned centers and develop professional relationships with case managers and discharge planners, hospital employees and management, patients, and families
- Complete clinical reviews via Allscripts and/or onsite evaluation as needed or requested, and coordinate a seamless transition to assigned centers
- Remain visible as assigned hospital and be available as a resource to promote Stonerise in a positive manner
- Abide to all HIPPA laws and observe confidentiality policy
- Present the benefits of long and short-term skilled nursing at Stonerise to patients, families, hospital employees, etc.
Requirements
- Two (2) years of experience in sales and marketing
- Current and valid driver's license with available transportation and ability to drive
