About the role
- Responsible for all staff and functions of Hope PACE.
- Analyze situations, problem solve and facilitate communications within the team and between other departments.
- Serve as a liaison between individual program disciplines, other PACE departments and community service providers.
Requirements
- Bachelors Degree from four-year college or university; or four years related experience and/or training; or equivalent combination of education and experience.
- Must possess a valid Florida Drivers' License, current auto insurance, and reliable transportation.
- If RN, must possess a current Florida license to practice nursing.
- Management of Hope Connections program – including all locations and staff.
- Active BLS for healthcare professionals from the American Heart Association or Red Cross; Current employees must obtain by January 1, 2025.
- One year supervisory experience preferred.
- One year elder care work experience desired.
- Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs.
