💰 $40,000 - $52,000 per year
About the role
- Support families on caseload with English-Spanish interpretation and prepare written reports and Personal Plans in both English and Spanish
- Provide families with information about the EI Services System, including information regarding choices and options of available services and supports to meet the child’s needs
- Work collaboratively as a team with the family and service providers to support children enrolled in the EI Program. Coordinate and ensure completion of initial and annual evaluations and assessments
- Facilitate and participate in developing the Individual Family Services Plan (IFSP) to support the child and assist the family in monitoring IFSP implementation and progress
- Provide ongoing advocacy support to ensure that appropriate EI services are being provided by communicating monthly with the family, using a variety of mechanisms, including face-to face visits
- Maintain accurate documentation in the form of case notes
- Facilitate the development of a transition plan to preschool, school, or if appropriate, to other services.
Requirements
- Bachelor’s Degree in Human Services or a related field (Required)
- Birth to age 5 Case Management experience (Preferred)
- Minimum one-year experience working with IDD and/or disabled population (Preferred)
- Early Intervention Credentialed (Preferred)
- Early Intervention Credential (Completion Required within 3 months of hire date)
- Valid Driver’s License, Insurance, and reliable transportation (Required)
- Ability to meet Essential Job Functions (with or without reasonable accommodation) (Required)
