- Plan and administer prescribed physical therapy treatment for patients in their homes
- Assess and document patients' physical strengths to support physician evaluation
- Develop and implement rehabilitation programs

Requirements
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
