Community Manager

• Support the rollout of RRR pilot programs in Eastern Kentucky by developing communication materials (i.e., press releases, one-pagers, flyers) to support partner awareness, community understanding, and consistent messaging across RRR components.
• Coordinate with implementation specialists and pilot sites (e.g., ARH Hazard, King’s Daughters) to deliver clear, locally relevant rollout communications that support adoption.
• Nurture strong relationships with key partners across Eastern Kentucky by spending extensive time in communities to establish trust with hospitals, CMHCs, clinical directors, and EMS directors.
• Actively participate in regional coalitions, forums, and workgroups related to rural health and behavioral health; attend standing meetings such as regional healthcare consortiums, mental health advocacy chapter meetings, and EMS regional alliance meetings as RRR’s representative.
• Center lived experience and community voice in RRR design and implementation, including engaging peers, CHWs, individuals with prior crisis episodes, and community-based organizations to inform program design and continuous improvement.
• Build and broker cross‑sector relationships and coalitions to engage diverse community leaders, strengthen trust, address social drivers of health, and support sustainable crisis response efforts.
• Serve as the primary liaison between RRR leadership and local communities by coordinating with state agencies (i.e., Department for Public Health, Department for Behavioral Health, Developmental, and Intellectual Disabilities) to align with statewide initiatives.
• Assess and support partnership/system readiness across pilot communities, including stakeholder alignment, sustainability planning, and community acceptability of program design – with attention to local context, cultural considerations, and network capacity.
• Organize periodic calls or meet-ups for cross-collaboration (i.e., ARH Hazard’s EmPATH unit can share lessons learned with leadership at King’s Daughters as they prepare their own unit).
• Help tailor RRR tools and materials so they are personalized for each, Eastern Kentucky partner; provide feedback to the RRR team on any training or toolkits that may need adjusting for local context.
• Support the integration of RRR services with existing local systems (i.e., work with 911 dispatch centers in Eastern counties to identify and divert calls to community paramedicine teams rather than law enforcement).
• Use regular engagement with communities to spot systemic issues or gaps early and convene the right partners to solve them.
• Maintain an Eastern region dashboard tracking community engagement and local ownership of RRR components, using insights to support evaluation and continuous improvement of all components.
• Conduct debriefs with local teams to learn what’s working and what barriers remain in the Eastern region.

Community Engagement Specialist

Implementation Specialist

• Lead identification of high-need rural hospitals for new EmPATH units and ensure each pilot site is committed and ready for implementation.
• Guide each pilot hospital through EmPATH unit design and launch, including facility preparations, staffing plans, and workflow to set up a safe unit in line with the EmPATH model.
• Support standardized clinical protocols/operations for EmPATH units, maintaining fidelity to evidence-based practices while complying with behavioral health regulations and safety standards.
• Oversee the RFA, application, and selection process, as well as contract management with participating hospitals.
• Develop referral and transfer workflows to support seamless handoffs from first responders and community paramedicine teams into EmPATH units.
• Partner with the Telebehavioral Health Hub Implementation Specialist to connect EmPATH units with the Telebehavioral Health Hub for coordinated, on-demand access to remote behavioral health providers across crisis response settings.
• Align EmPATH unit operations with CMHCs to support post-crisis care, so stabilized patients have warm hand-offs to ongoing care.
• Partner with experienced EmPATH providers to deliver coordinated training for new unit teams, covering psychiatric emergency assessment, de‑escalation, workflows, and data/reporting tools.
• Organize on-site coaching peer observation opportunities at established EmPATH units and connect sites with EmPATH experts for best practices.
• Share key updates, challenges, and input on implementation strategies with the broader EmPATH Consortium, a cross-sector advisory group to guide statewide EmPATH expansion.
• Serve as the primary liaison between RRR leadership and local communities by coordinating with state agencies (i.e., Department for Public Health, Department for Behavioral Health, Developmental, and Intellectual Disabilities) to align EmPATH implementation with statewide crisis initiatives such as 988.
• Define and monitor key performance metrics for EmPATH. Tracking data should include stabilization/transfer rates in EmPATH units, follow-up care engagement, etc.
• Analyze performance data to identify trends, gaps, and success stories to evaluate whether EmPATH units are decreasing ED boarding and increasing connection to community care.

Rapid Response to Recovery EmPATH Implementation Specialist

• Guide the selection, onboarding, and readiness assessment of KY EMS agencies to start or expand community paramedicine teams.
• Oversee planning and launch of EMS-led behavioral health crisis response services, including staffing, training, protocol development, and operational logistics.
• Ensure fidelity to the RRR model and compliance with the Kentucky Board of Emergency Medical Services (KBEMS).
• Oversee data interoperability enhancements.
• Develop dispatch and referral workflows with 911 and 988 systems to enable timely deployment of community paramedicine team.
• Coordinate with EmPATH units and the Telebehavioral Health Hub to ensure seamless handoffs and integrated care pathways.
• Align operations with CMHCs/CCBHCs and other behavioral health providers to support warm handoffs and post-crisis follow-up.
• Coordinate and support behavioral health-focused training for community paramedics by facilitating partnerships with CMHCs, and ensuring content addresses de-escalation, overdose response, trauma-informed care, and crisis stabilization.
• Track registrations and agency attendance to ensure required staff complete CMHC-delivered trainings.
• Maintain toolkits, protocols, and reference materials provided by CMHCs, to guide EMS agencies through implementation and operations.
• Build and help manage partnerships across first responders, behavioral health providers, and other cross-sector crisis response partners – including KBEMS, EMS agencies, CMHCs, and local leaders – to support program success and sustainability.
• Serve as the primary liaison for community paramedicine efforts in Kentucky, representing RRR in regional coalitions.
• Facilitate MOUs and collaborative agreements to support data sharing, referral pathways, and coordinated care.
• Define and track key performance metrics such as ED diversion rates, EMS response times, and follow-up engagement.
• Analyze data to identify trends, gaps, and success stories to lead quality improvement efforts across pilot sites.
• Prepare reports to communicate program impact to RRR leadership, CMS, and other community partners.

Community Paramedicine Implementation Specialist

• Guide the design, partner coordination, and deployment of the statewide Telebehavioral Health Hub
• Oversee platform build, testing, and integration with EHRs, KHIE, and care coordination systems while helping manage technology procurement and site readiness.
• Coordinate phased rollout and onboarding of pilot sites, ensuring 24/7 clinical coverage and compliance with telehealth regulations.
• Develop workflows to embed the hub into 911/988 dispatch, primary care clinics, EmPATH units, EMS, and CMHC operations for real-time psychiatry support.
• Incorporate prevention and post‑crisis (postvention) supports into hub workflows.
• Align hub operations with statewide crisis services and ensure seamless data exchange and referral tracking across systems.

• Conduct key informant (stakeholder) interviews to gather information related to Agency performance and response metrics related to the measles outbreak response of 2025/2026.
• Review documentation related to the measles outbreak response of 2025/2026, including after-action review submissions by Incident Management Team personnel, to produce a timeline of events and lessons learned for the response.
• Compile interview materials and information and assist in the drafting of a full after-action report detailing Agency timelines, decisions, and operational information related to the measles outbreak response.
