About the role
- serves as a primary liaison between the Carle Health organization and external clinical partners
- supports EpicCare Link and Community Connect initiatives
- manages contracting of all new EpicCare Link sites (clinical, SNFs and ALFs)
- collaborates with cooperative nursing and leadership, external providers, internal providers, IT-EHR, Business Development, Social Work, Radiology, and Health Information Management to support external partners
- leads, manages and/or establishes Regional Work Group meetings across all regions
- supports ongoing reporting requirements for Community Connect partners
- validates HIPPA training has been completed for all new SNFs and ALFs
- develops and maintains knowledge base of current Carle provider specialists and the breadth and depth of Carle specialty services
Requirements
- Associate's Degree
- Work Experience: Healthcare
