About the role
- Provide clinical operations support to achieve quality care
- Identify and triage members for appropriate referrals
- Coordinate care across a large regional area
- Support strategic needs of the Care Management Team
- Collaborate with community partners and maintain relationships
Requirements
- Minimum 3 years’ experience in healthcare or social services
- Experience working with vulnerable populations
- Knowledge of Medicaid and Medicare
- Licensed practical nurse (LPN), certified medical assistant (CMA), or certified nurse’s assistant (CNA) preferred
- Familiarity with health plan claims systems
- Proficient in Microsoft Office
- Skills in data analysis and using analytical tools (Tableau, Power BI)
