Provide Organizational Change Management (OCM) services, consultation, and support for a client Case Management project.
Engage with internal and external stakeholders, including client staff and external providers.
Document project goals and objectives; conduct a baseline assessment and gap analysis.
Develop a Change Management Action/Communication Plan for the change initiative.
Facilitate kickoff meetings and regular check-ins with stakeholders.
Measure the success of the change initiative and lead corrective action activities to achieve success.
Document all OCM documentation, templates, forms, etc. that are a result of this agreement, and associated deliverables.

Requirements

Within the last ten (10) years, must have at least five (5) years of experience applying analytical processes on IT projects.
Within the last ten (10) years, at least three (3) years of applying analytical processes on IT Projects experience must have been in business systems analysis and design.
Must possess a Bachelor’s Degree.
Within the last ten (10) years, must have three (3) years of working knowledge and experience applying Organizational Change Management methodologies to an enterprise-wide IT project.
Must be able to provide a client/user end reference contact information for each applicable project meeting the requirements upon request in the qualification form.
In addition, you must be able to submit completed reference forms when requested. The reference contact must be a representative of the company for which the project was developed and must have served in a management or supervisory role.
Within the last five (5) years, a minimum of three (3) years of working knowledge and experience with implementation and deployment of a new software system within the Health and Human Services field.
Possess a current Organizational Change Management Certification.
