About the role
- Provide one-to-one assessments and develop personalised support plans
- Help people access mental health services and community activities
- Identify and address wider needs, such as housing, finances, and employment
- Maintain records and monitor outcomes for all service users
- Work collaboratively with mental health and community partners
- Promote PBHA participation opportunities and adhere to safeguarding procedures
- Organise own workload and comply with PBHA policies
