About the role
- Assess patient eligibility for specific service needs, developing individualized integrated action plans
- Refer and connect patients to resources, monitoring adherence and providing necessary follow-ups
- Complete detailed reports on needs assessment and case management activities
- Provide crisis and de-escalation techniques
- Attend required meetings, including supervision, treatment team meetings, and quarterly in-service
Requirements
- 1 year of experience working with adults in a human service setting
- Prior experience with harm reduction (preferred)
- Bachelors Degree in a related Behavioral Science field
