About the role

  • Assess patient eligibility for specific service needs, developing individualized integrated action plans
  • Refer and connect patients to resources, monitoring adherence and providing necessary follow-ups
  • Complete detailed reports on needs assessment and case management activities
  • Provide crisis and de-escalation techniques
  • Attend required meetings, including supervision, treatment team meetings, and quarterly in-service

Requirements

  • 1 year of experience working with adults in a human service setting
  • Prior experience with harm reduction (preferred)
  • Bachelors Degree in a related Behavioral Science field
