About the role

  • Assess patient eligibility for specific service needs, developing individualized integrated action plans
  • Refer and connect patients to resources, monitoring adherence and providing necessary follow-ups
  • Complete detailed reports on needs assessment and case management activities
  • Provide crisis and de-escalation techniques
  • Ensure patients have reliable transportation to BrightView and other care providers
  • Attend required meetings, including supervision, treatment team meetings, and quarterly in-service
  • Adhere to regulatory and company requirements in documentation and evaluation
  • Participate in TCM audit, meeting assessment and treatment plan completion expectations

Requirements

  • 1 year of experience working with adults in a human service setting
  • Bachelors Degree in a related Behavioral Science field
  • Familiarity with MS Office software (Outlook, Teams, Word, etc) preferred
  • Familiarity with HIPAA and 42-CFR Part 2 desired
  • Prior experience with harm reduction a plus
