About the role

  • Works collaboratively with pharmacists, nurses, physicians, and leadership to develop programs that improve patient outcomes
  • Developing and implementing anticoagulation policies and procedures
  • Serving as a member of the local anticoagulation services committee
  • Developing ongoing competency monitoring
  • Assisting with staff scheduling to ensure appropriate coverage
  • Assessing appropriate drug information and literature resources
  • Identifying and engaging in department quality improvement activities
  • Participating as a preceptor for the LHMC PGY-1 and PGY-2 residency programs
  • Training new staff in all areas of anticoagulation clinic

Requirements

  • Advanced degree (PharmD) or equivalent clinical practice experience
  • License Massachusetts Pharmacist required or immediate eligibility
  • 3-5 years related work experience required
  • Must demonstrate effective verbal and written communication skills
  • Demonstrate computer literacy skills including web-based applications and some Microsoft Office applications
