About the role
- Assumes individual case load which involves conducting individual intake evaluation and counseling sessions
- Providing referral to continuing care
- Sharing responsibility for group programming
- Obtaining and documenting all relevant intake material
- Assesses patients’ continuing care needs and arranges for transfer or admission to rehabilitation facilities
- Provides referrals for affected family members to self-help programs
- Supports client’s transportation to the next level of care when appropriate
- Supervises the health and safety of patients by completing trauma-informed checks
- Maintains all required documentation including observation logs and resident belongings inventories
Requirements
- A minimum of one year related experience and/or training in the area of addiction treatment
- Associates Degree in Human Services or related field preferred
- HS diploma or equivalent required
- Valid driver's license in good standing preferred
- Must be able to complete on-site CPR training/certification
- Must demonstrate ability to relate to a substance abuse treatment population in a sensitive manner
- An interest and willingness to work in a multicultural setting and must share a customer service orientation.
