About the role
- Provides physical therapy services to patients according to a written physician's plan of care; assesses functional status and contributes to the total plan of care
- Evaluates home environment and assesses strength, mobility, gait, ROM
- Instructs patients and families in the use and care of therapeutic interventions
- Determines priority needs for physical therapy; reports patient’s reaction to treatment or changes in condition to the physician
- Initiates physical therapy program and trains other personnel/family members
- Trains patients in prosthetic use as applicable
- Identifies needs for additional home health services and makes referrals
- Participates in discharge planning; prepares clinical progress summaries per policy
- Provides PT consultation to home families/caregivers as needed
- Delivers in-service education and participates in peer consultation
- Supervises PT assistants and certified OT assistants per policy and state
Requirements
- Licensed Physical Therapist in California (PT) or eligible to practice
- New grads welcome
- Ability to assess muscle strength, mobility, gait, ROM
- Ability to develop and implement plan of care per physician's plan
- Ability to evaluate home environment
- Ability to instruct patients and families/caregivers in therapy
- Strong communication with physicians and care teams
- Supervisory experience with PT assistants and OT assistants (as per policy)
- Ability to coordinate discharge planning and referrals to home health services
- Commitment to patient safety and documentation
