Provides physical therapy services to patients according to a written physician's plan of care; assesses functional status and contributes to the total plan of care
Evaluates home environment and assesses strength, mobility, gait, ROM
Instructs patients and families in the use and care of therapeutic interventions
Determines priority needs for physical therapy; reports patient’s reaction to treatment or changes in condition to the physician
Initiates physical therapy program and trains other personnel/family members
Trains patients in prosthetic use as applicable
Identifies needs for additional home health services and makes referrals
Participates in discharge planning; prepares clinical progress summaries per policy
Provides PT consultation to home families/caregivers as needed
Delivers in-service education and participates in peer consultation
Supervises PT assistants and certified OT assistants per policy and state
Requirements
Licensed Physical Therapist in California (PT) or eligible to practice

Ability to assess muscle strength, mobility, gait, ROM
Ability to develop and implement plan of care per physician's plan
Ability to evaluate home environment
Ability to instruct patients and families/caregivers in therapy
Strong communication with physicians and care teams
Supervisory experience with PT assistants and OT assistants (as per policy)
Ability to coordinate discharge planning and referrals to home health services
