Therapist

• Provides physical therapy services to patients according to a written physician's plan of care; assesses functional status and contributes to the total plan of care
• Evaluates home environment and assesses strength, mobility, gait, ROM
• Instructs patients and families in the use and care of therapeutic interventions
• Determines priority needs for physical therapy; reports patient’s reaction to treatment or changes in condition to the physician
• Initiates physical therapy program and trains other personnel/family members
• Trains patients in prosthetic use as applicable
• Identifies needs for additional home health services and makes referrals
• Participates in discharge planning; prepares clinical progress summaries per policy
• Provides PT consultation to home families/caregivers as needed
• Delivers in-service education and participates in peer consultation
• Supervises PT assistants and certified OT assistants per policy and state
