About the role
- Provides, facilitates and supervises patient centered care and care coordination to ensure patient care needs are met while maintaining quality standards and regulatory compliance in a financially responsible manner.
- Serves as a resource to patients, families, physicians, and staff by providing hands on care, clinical support, managerial support, interpreting policies, and facilitating care.
- Hires, leads, mentors, develops, and evaluates staff in a manner that promotes engagement, productivity, and ensures competencies.
- Supervises and delivers care based on the best evidence available and may participate in research activities within clinical practice.
- Ensures effective use of resources, equipment and supplies.
Requirements
- Graduate of an accredited program in Occupational, Physical, Audiology or Speech Therapy required.
- Must hold a current license or certification in respective discipline.
- A minimum 2 years of Home Health experience required.
