About the role
- Assess, document, and monitor overall health status of assigned members
- Develop service plans based on member assessments
- Authorize and coordinate with provider agencies
- Facilitate and advocate for members with service providers
Requirements
- Bachelor’s degree in social work
- 2 years of experience serving elderly persons or persons with physical disabilities or Serious Mental Illness (SMI)
- Bilingual preferred in some assignments
