About the role

  • Support the development of a comprehensive supportive treatment plan
  • Coordinate physical, emotional, mental, spiritual, and social health services
  • Make appropriate referral and linkages to community services and resources
  • Provide services in a supportive, confidential, and culturally safer manner to help clients work towards their goals and access the resources they need to fully participate in their health care services

Requirements

  • Completion of a Social Service Worker Program and 2 years experience; or an equivalent combination of education and experience
  • Completion of training in case management considered an asset
  • Demonstrated experience working with Indigenous communities and clients; ability to provide culturally safe care
  • Experience providing case management services to clients with complex needs in the social determinants of health, particularly homelessness, mental health and addictions, and transportation.
  • Case Manager skills outlined in the Canadian Core Competency Profile for Case Management Providers (2012).
  • Demonstrated knowledge of health, social and community-based resources required to provide case management services
  • Communication skills to explain the case management process to clients and collaborators including the processes of screening, assessing, planning, implementing, evaluating, and transitioning.
  • Case management assessment skills to develop an understanding of client needs in physical, psychosocial, emotional, spiritual and cultural domains
  • Team work with interprofessional team members internal to an organization and external to an organization
  • Critical thinking and application skills to anticipate client needs and develop a strong Case Management Plan
  • Relationship building skills with clients, social networks, and community service providers
  • Trauma-informed practice skills applied to supporting clients with complex social determinants of health needs
  • Decision-making skills and demonstrated experience applying decisions to actions to support clients .
