Manage a Patient Panel: Oversee the behavioral health journey for patients enrolled in the BHI program.
Analyze Rich Data: Review comprehensive patient data packages delivered by the Amplifier platform to assess progress, identify risks, and inform care plan adjustments.
Develop & Update Care Plans: Create, review, and modify person-centered behavioral health care plans in collaboration with primary care providers.
Ensure Compliance and Quality: Meticulously document clinical activities to meet CPT 99484 reimbursement requirements and maintain care standards.
Collaborate with Clinical Partners: Serve as key clinical point of contact, providing updates and coordinating with primary care providers for integrated care.
Provide Patient Support: Engage directly with patients for support, clinical check-ins, and guidance as outlined in care plans.
Practice at top of license using AI-native platform to focus on clinical judgment rather than administrative tasks.
Requirements
Active, unrestricted license as RN, LCSW, LPC, LMFT, LPN, or Certified/Registered Medical Assistant (CMA/RMA) with significant behavioral health experience.
Minimum of 2 years of experience in behavioral health, psychiatric care, case management, or a related field.
Comfortable and enthusiastic about using new technologies, software platforms, and digital health tools.
Strong knowledge of Behavioral Health Integration (BHI), the Collaborative Care Model (CoCM), and measurement-based care principles.
Exceptional documentation skills with ability to produce clear, concise, and compliant clinical documentation.
Self-starter, highly organized, and proven ability to work effectively in a fully remote environment.
