About the role
- Conduct assessments to identify patient needs, plan and implement interventions, and evaluate patient progress
- Provide comprehensive occupational therapy services in accordance with physician orders
- Develop and modify treatment plans in collaboration with the patient, caregiver, and physician
- Educate patients and caregivers on proper techniques to promote independence with ADL's
- Maintain accurate and timely documentation of patient progress
- Communicate with other healthcare professionals involved in patient care to ensure effective treatment planning
Requirements
- Degree from an accredited Occupational Therapy program
- Current Occupational Therapy license in the state of Michigan
- At least one year of experience in home health care setting is preferred
- Strong critical thinking and problem-solving skills
- Excellent communication and interpersonal abilities
- Ability to work independently and in coordination with a team
- Reliable transportation and valid driver's license
