Lead CHW program planning and evaluation activities, and monitor student training along with other program developmental trainings.
Lead CHW program didactics.
Support successful design and delivery of CHW programs by engaging internal / external experts to ensure high impact strategies, structures, and inclusion of best knowledge available.
Manage CHW project, helping to ensure that required tasks are completed, risks to program deliverables are reported and mitigated, and open issues are resolved with minimal disruption to project timelines.
Attend and participate in local, county, regional, and statewide meetings, committees, or conferences as required by program goals and objectives.
Research, design and recommend opportunities to enhance CHW pilot efforts through scaling best practices, technology platforms and ideas generated by impacted communities.
Effectively prioritize projects to complete work within established guidelines and timeframe.
Develop and implement a strong career readiness curriculum that prepares CHW students at Central Health.
Facilitate teambuilding activities to foster team spirit and team rapport by creating a positive CHW group dynamic.
Support patient education initiatives.
Complete other duties as assigned.
Requirements
Bachelor's Degree - Required
Master's Degree (Public health, social work, psychology, or related field) - Preferred
4 years as a Community Health Worker - Required
4 years of demonstrated experience and understanding of health-related social determinants and chronic diseases prevalent in a primary care population (either through work or school) - Required
2 years of experience in organizing and developing new programs related to business needs - Required
2 years of experience in group facilitation (e.g., teaching of workshops or courses) - Preferred
2 years of direct experience working with patients or clients (in a healthcare or non-profit setting) - Preferred
2 years of experience supervising or leading others - Preferred
Required to reside in Austin, TX and surrounding areas and be on-site four days per week and as needed to serve as the primary facilitator for the CHW program curriculum.
