Care Coordinator

• Serve as the first point of contact for prospective clients via phone, email, and text
• Provide exceptional customer service and a welcoming experience
• Coordinate the new client onboarding process with the Care Services Manager
• Schedule consultations and match clients with clinicians
• Coordinate insurance matters with the billing team
• Set up client portals and manage intake documentation
• Maintain accurate client records and create client profiles in Simple Practice
• Communicate with clients, clinicians, and internal team members to support seamless onboarding
• Handle client transfers, cancellations, reschedules, and clinician transitions
• Collaborate with the leadership team to support client care, workflow efficiency, and administrative initiatives
