About the role

Key responsibilities & impact

Triage incoming patient referrals, assess patient needs, provide clinical dispositions, and facilitate referrals to primary and specialty care providers
Educate patients about immediate care advice and preventive behaviors
Accompany patients to appointments and coordinate travel, accommodations, and itineraries
Assess and coordinate multidisciplinary care across the care continuum
Collaborate with care teams to expedite patient workups, treatment, survivorship, and end-of-life care
Schedule and coordinate clinic, e-visit, and telehealth appointments
Communicate clinical information between patients and providers
Assess health literacy and educational needs and provide clinical education
Document patient needs, goals, preferences, conversations, and care plans in the medical record
Identify care gaps, troubleshoot transitions and barriers to care, and communicate with referring and primary care providers
Provide direction and guidance to Patient Navigators
Participate in service line team meetings

Requirements

What you’ll need

Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse
Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) is acceptable
Three (3) years of registered nursing experience
Ability to handle and maintain confidential information
Ability to work in a fast-paced and rapidly changing environment
Excellent oral and written communication skills
Strong organizational and motivational skills
Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, and grasping
Ability to sit for extended periods of time
Ability to work primarily 7a-7p, with 7p-7a and weekend rotation
Basic Life Support certification is listed as preferred, with ability to obtain within 30 days of hire
