Serve as a trusted community member linking health, social services, and members to increase access and improve quality of services
Integral part of the member’s care team; work closely with the Case Manager
Lead the Connect For Life program using Wider Circle’s approved curriculum and materials to build trust, promote health, and encourage engagement
Plan, promote, and host virtual and in-person events for members; foster relationships to support wellness outcomes
Regularly reach out to members by phone and in-person to check in, identify and address needs, and encourage event participation
Conduct home visits as needed to support members, provide resources, and encourage program compliance with care plans
Establish and maintain partnerships with local community-based organizations to bring resources to members
Record details of interactions with members in the case management system (CMS) using a computer, tablet, or smartphone
Be accountable for achieving membership growth targets, engagement and retention goals; monitor success by tracking attendance, feedback, and metrics
Identify member leaders and invite them to serve as Connect For Life Ambassadors to assist in organizing meetings and outreach
Requirements
Certified Community Health Worker
Have Enhanced Care Management or complex care management experience
Have 3+ years of relevant community outreach, teaching, facilitation, volunteer, or healthcare experience
Embrace a remote team working environment, working independently with little oversight
Understand the socio-economic and public health challenges facing disenfranchised people
Have excellent presentation and communication skills, both written and verbal, and be comfortable speaking one-on-one and to larger groups
Have an outgoing personality and unwavering, positive attitude, with the ability to comfortably engage with others in-person and via video conference or phone
Love building relationships and networking and have a knack for motivating and influencing different types of people
Great at organizing, prioritizing, and following through on commitments
Have strong computer skills and the ability to navigate web-based and app-based systems
Have reliable transportation and be comfortable traveling to members’ homes
Willing to commute 20 - 40 miles
Have a flexible schedule and be able to work outside of regular business hours and when necessary
Be committed to a drug-free workplace and ready for pre-employment substance abuse testing and background checks
