About the role
- Meet members at their homes to introduce and explain our programs.
- Discuss and promote the benefits of ECM and CalAIM services.
- Facilitate in-person Connect for Life activities focusing on trust-building, health promotion, and movement.
- Build and nurture relationships with members through regular follow-up, both in person and over the phone.
- Encourage members to participate in community meetings and recruit new members to join.
- Serve as a liaison between members and their health plans, ensuring they understand their benefits and options.
- Partner with local community organizations to offer additional services and resources to our members.
Requirements
- Bilingual (English/Spanish) preferred with excellent communication skills.
- Strong empathy, compassion, and ability to connect with others.
- Experience working with Social Determinants of Health (SDOH) and low-income communities is preferred.
- Previous canvassing, sales, or community organizing experience preferred.
- Comfortable working independently, remotely, and in the field.
- Ability to engage one-on-one, build rapport, and motivate others.
- Familiarity with applications and software; ability to work with remote technology.
- Reliable transportation to travel extensively to members’ homes and community events.
- Flexibility to work after-hours, weekends, and outdoors.
- Ability to stand and walk for long periods.
- Safely and consistently drive to public places up to 100 miles
