About the role

  • Oversee a portfolio of Community Health programs with medium-high complexity
  • Manage and develop key community partnerships to advance program reach and portfolio
  • Facilitate presentations to service line leaders and community partners on program impact
  • Leads and facilitates communication and interaction among program stakeholders
  • Develop relationships with stakeholders including residents and attend community meetings and outreach events

Requirements

  • Bachelors in Business Administration/Management, Public Health, or equivalent
  • Minimum 5 years of experience in community health and social services, project management or grant management experience or 2 years with a Master’s Degree
  • High level of organization and attention to detail
  • Strong critical thinking and analysis, planning and problem-solving skills
  • Familiarity with program and grant implementation
