About the role
Key responsibilities & impact- Provide services throughout the Western Maine area
- Provide support to adults with intellectual disabilities/autism and their families
- Advocate for individuals and ensure choice and satisfaction with services
- Monitor individuals’ health and safety
- Assist individuals and their teams to identify needs and develop plans of care
- Initiate referrals and monitor services
- Work closely with the multi-disciplinary team
- Coordinate and advocate for services identified in the plan
- Monitor progress and evaluate the effectiveness of services
Requirements
What you’ll need- BA/BS in human services or closely related field, or BA/BS in unrelated field plus a minimum of 1 year experience in adult IDD services
- Knowledge of the system of care in Maine preferred
- Reliable vehicle with proper insurance
- Valid driver’s license issued by state of residence with clean driving record
- Proficient in speaking, reading, and writing English
- Strong interpersonal and communication skills
- Ability to complete agency and state mandated training programs with proficiency
