About the role

Key responsibilities & impact
  • Provide services throughout the Western Maine area
  • Provide support to adults with intellectual disabilities/autism and their families
  • Advocate for individuals and ensure choice and satisfaction with services
  • Monitor individuals’ health and safety
  • Assist individuals and their teams to identify needs and develop plans of care
  • Initiate referrals and monitor services
  • Work closely with the multi-disciplinary team
  • Coordinate and advocate for services identified in the plan
  • Monitor progress and evaluate the effectiveness of services

Requirements

What you’ll need
  • BA/BS in human services or closely related field, or BA/BS in unrelated field plus a minimum of 1 year experience in adult IDD services
  • Knowledge of the system of care in Maine preferred
  • Reliable vehicle with proper insurance
  • Valid driver’s license issued by state of residence with clean driving record
  • Proficient in speaking, reading, and writing English
  • Strong interpersonal and communication skills
  • Ability to complete agency and state mandated training programs with proficiency
