About the role
Key responsibilities & impact- Support children and adolescents from birth to 21 years of age with developmental disabilities and/or behavioral health needs
- Work with child's family/caregivers to assess and identify needs
- Develop an individualized plan of care
- Organize services and supports
- Collaborate with Children's Care Management Team including Nursing, Psychiatry, Medical, and Family Supports providers
Requirements
What you’ll need- BA/BS in human services or closely related field or
- BA/BS in unrelated field + minimum 1 year experience in relevant children’s services
- Knowledge of the system of care in Maine preferred
- Reliable vehicle with proper insurance
- Valid driver’s license issued by state of residence
- Strong interpersonal communication skills, proficiency with written, spoken English, and ability to utilize various technology systems for record keeping/plan monitoring.
