About the role

Key responsibilities & impact
  • Support children and adolescents from birth to 21 years of age with developmental disabilities and/or behavioral health needs
  • Work with child's family/caregivers to assess and identify needs
  • Develop an individualized plan of care
  • Organize services and supports
  • Collaborate with Children's Care Management Team including Nursing, Psychiatry, Medical, and Family Supports providers

Requirements

What you’ll need
  • BA/BS in human services or closely related field or
  • BA/BS in unrelated field + minimum 1 year experience in relevant children’s services
  • Knowledge of the system of care in Maine preferred
  • Reliable vehicle with proper insurance
  • Valid driver’s license issued by state of residence
  • Strong interpersonal communication skills, proficiency with written, spoken English, and ability to utilize various technology systems for record keeping/plan monitoring.
