About the role

Key responsibilities & impact

Supports children and adolescents from birth to 21 years of age with developmental disabilities and/or behavioral health needs
Works with the child's family/caregivers to assess and identify needs
Develops an individualized plan of care
Organizes services and supports
Is a member of the Children’s Care Management Team that includes Nursing, Psychiatry, Medical, and Family Supports providers

Requirements

What you’ll need

BA/BS in human services or closely related field or
BA/BS in unrelated field + minimum 1 year experience in relevant children’s services
Knowledge of the system of care in Maine preferred
Reliable vehicle with proper insurance
Valid driver’s license issued by state of residence
Strong interpersonal communication skills, proficiency with written, spoken English, and ability to utilize various technology systems for record keeping/plan monitoring.
