About the role
Key responsibilities & impact- Provide comprehensive nursing care and support to geriatric patients in accordance with established clinical guidelines, policies, and standards of practice
- Conduct telephone triage assessments via incoming and outgoing calls
- Evaluate patient symptoms and concern, provide appropriate nursing advice, and escalate urgent issues to providers when necessary
- Manage incoming and outgoing patient phone communications, phone triage, including appointment scheduling, follow-up coordination, and communication of test results and provider recommendations
- Review, prioritize, and respond to patient portal (MyChart) messages in a timely and professional manner
- Prepare, pend, and process medication refills, laboratory orders, diagnostic testing orders, referrals, and other provider-directed orders following established protocols
- Review patient medical records and clinical documentation to ensure appropriate medication refill requests and continuity of care
- Complete and process a variety of patient-related forms and documentation
- Facilitate electronic document completion and signature workflows through DocuSign and other approved electronic platforms
- Request, obtain, review, and maintain external medical records to support ongoing patient care, treatment planning, and provider review
- Coordinate communication among patients, caregivers, providers, specialists, pharmacies, and community resources to ensure continuity of care
- Educate patients and family members regarding treatment plans, medications, disease management, preventive care, and available healthcare resources
- Maintain accurate, complete, and timely documentation within the electronic health record (EHR) system
- Assist with care coordination, chronic disease management, and follow-up for geriatric patients with complex medical needs
- Ensure compliance with HIPAA regulations, organizational policies, and applicable state and federal healthcare requirements
- Participate in quality improvement initiatives and support a patient-centered approach to care delivery
Requirements
What you’ll need- Graduate of an accredited nursing program
- Current RN licensure in the state of Missouri and/or Illinois, depending on location, in accordance with Compact Act
- Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements)
