About the role

Key responsibilities & impact

Provide comprehensive nursing care and support to geriatric patients in accordance with established clinical guidelines, policies, and standards of practice
Conduct telephone triage assessments via incoming and outgoing calls
Evaluate patient symptoms and concern, provide appropriate nursing advice, and escalate urgent issues to providers when necessary
Manage incoming and outgoing patient phone communications, phone triage, including appointment scheduling, follow-up coordination, and communication of test results and provider recommendations
Review, prioritize, and respond to patient portal (MyChart) messages in a timely and professional manner
Prepare, pend, and process medication refills, laboratory orders, diagnostic testing orders, referrals, and other provider-directed orders following established protocols
Review patient medical records and clinical documentation to ensure appropriate medication refill requests and continuity of care
Complete and process a variety of patient-related forms and documentation
Facilitate electronic document completion and signature workflows through DocuSign and other approved electronic platforms
Request, obtain, review, and maintain external medical records to support ongoing patient care, treatment planning, and provider review
Coordinate communication among patients, caregivers, providers, specialists, pharmacies, and community resources to ensure continuity of care
Educate patients and family members regarding treatment plans, medications, disease management, preventive care, and available healthcare resources
Maintain accurate, complete, and timely documentation within the electronic health record (EHR) system
Assist with care coordination, chronic disease management, and follow-up for geriatric patients with complex medical needs
Ensure compliance with HIPAA regulations, organizational policies, and applicable state and federal healthcare requirements
Participate in quality improvement initiatives and support a patient-centered approach to care delivery

Requirements

What you’ll need

Graduate of an accredited nursing program
Current RN licensure in the state of Missouri and/or Illinois, depending on location, in accordance with Compact Act
Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements)
