About the role

Key responsibilities & impact
  • Provide comprehensive nursing care and support to geriatric patients in accordance with established clinical guidelines, policies, and standards of practice
  • Conduct telephone triage assessments via incoming and outgoing calls
  • Evaluate patient symptoms and concern, provide appropriate nursing advice, and escalate urgent issues to providers when necessary
  • Manage incoming and outgoing patient phone communications, phone triage, including appointment scheduling, follow-up coordination, and communication of test results and provider recommendations
  • Review, prioritize, and respond to patient portal (MyChart) messages in a timely and professional manner
  • Prepare, pend, and process medication refills, laboratory orders, diagnostic testing orders, referrals, and other provider-directed orders following established protocols
  • Review patient medical records and clinical documentation to ensure appropriate medication refill requests and continuity of care
  • Complete and process a variety of patient-related forms and documentation
  • Facilitate electronic document completion and signature workflows through DocuSign and other approved electronic platforms
  • Request, obtain, review, and maintain external medical records to support ongoing patient care, treatment planning, and provider review
  • Coordinate communication among patients, caregivers, providers, specialists, pharmacies, and community resources to ensure continuity of care
  • Educate patients and family members regarding treatment plans, medications, disease management, preventive care, and available healthcare resources
  • Maintain accurate, complete, and timely documentation within the electronic health record (EHR) system
  • Assist with care coordination, chronic disease management, and follow-up for geriatric patients with complex medical needs
  • Ensure compliance with HIPAA regulations, organizational policies, and applicable state and federal healthcare requirements
  • Participate in quality improvement initiatives and support a patient-centered approach to care delivery

Requirements

What you’ll need
  • Graduate of an accredited nursing program
  • Current RN licensure in the state of Missouri and/or Illinois, depending on location, in accordance with Compact Act
  • Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements)
