About the role

  • Clinical Lead Care Manager (CLCM) serving as the client’s primary point of contact and coordinating with providers to manage client care
  • Manage client cases, coordinate health care benefits, provide education, and facilitate member access to timely and cost-effective care
  • Collaborate with caregivers, providers, and the Care Team to promote wellness, recovery, independence, resilience, and member empowerment
  • Hybrid position requiring travel throughout Sacramento County with both in-person and remote duties
  • Assess member needs across physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, social supports, housing, and community-based services and supports
  • Oversee development of client care plans and SMART goals; evaluate client progress and update goals
  • Offer services where the member resides or finds most accessible (office, telehealth, or field-based services) and conduct outreach to facilitate linkage to the ECM program
  • Advocate on behalf of the client with health care professionals and connect clients to social services and supports
  • Utilize evidence-based practices such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care
  • Maintain up-to-date patient health records in EMR and CRM; complete documentation, outcome measures, and monthly reporting; attend training as assigned

Requirements

  • Active California Licensed Vocational Nurse (LVN) license required
  • Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely
  • 2+ years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations
  • Working knowledge of government and community resources related to social determinants of health
  • Excellent oral and written communication skills
  • Positive interpersonal skills
  • Clean driving record, valid driver's license, and reliable transportation
  • General computer skills and a working knowledge of Google Workspace, MS Office, and the internet
  • COVID-19 and influenza vaccination required for patient-facing roles (religious/medical accommodations may be considered)
  • Background check and E-Verify employment eligibility verification required prior to hire
