About the role
Key responsibilities & impact- Provides general case management and referral services to all residents
- Establishes partnerships with public and private agencies for service referrals
- Conducts assessments for residents participating in the program
- Monitors the delivery of services to residents
- Documents contact with residents and maintains individual files
- Completes documentation and reports to comply with contractual agreements
- Performs other reasonable related duties as assigned
Requirements
What you’ll need- A bachelor’s degree in social work, Gerontology, Psychology, Counseling, Public Health, or Therapeutic Recreation is preferred
- At least two years of experience conducting supportive service needs assessments for the elderly
- Demonstrated working knowledge of services available in the community
- Training in the aging process, elder services, drug and alcohol abuse, and mental health issues
- Awareness of federal and state entitlement programs and legal liability issues
- A valid California driver license and ability to meet organizations insurance carrier guidelines
