About the role

Key responsibilities & impact

Responsible for the development of a case management plan to assist program participants.
Consult with staff for assessment, individual counseling, and group facilitation.
Perform daily work requirements to achieve established department objectives.
Provide written assessments of program participants’ needs and develop individualized service plans.
Evaluate and revise service plans on a regular basis.
Facilitate employment-focused groups.
Educate participants on living skills, budgeting, health and hygiene, and substance abuse.
Maintain comprehensive lists of community resources for referrals.
Document all contact with program participants and prepare necessary records.

Requirements

What you’ll need

A Bachelor’s Degree in Social Work or a related field is preferred.
Two years of successful experience in the delivery of human services.
Knowledge of social casework objectives, principles, and methods.
Knowledge of public welfare laws and public welfare agencies.
Knowledge of employment readiness, permanent/supportive housing, and community resources.
Skills in interviewing, cultural competency, and diversity principles.
Ability to demonstrate skill in the application of casework methods and practices.
Must pass background clearances in accordance with agency, County, State, and federal standards.
CPR/First Aid certification required within 90 days of employment.
