About the role

Key responsibilities & impact
  • Performs case management with individual program participants and families in the Parker House Program.
  • Functions at a highly skilled level in assessment, individual goal planning, group facilitation, community resource development, and file monitoring and review.
  • Develop successful outcomes for clients alongside Housing Specialists and Employment Specialists.
  • Continuous improvement of service quality through participant and stakeholder feedback.
  • Responsible for daily work requirements to achieve established objectives of the department.
  • Assist Case Managers, Housing Specialists, and Employment Specialists with client barriers.
  • Overall assessment of program participant’s needs and development of individualized service plans.
  • Educate program participants on various aspects such as living skills, parenting, financial capability, workforce development, health and hygiene, housing, and substance abuse.
  • Develop and maintain lists of community resources for program participants; refer them accordingly.
  • Document all participant contact and prepare all assessments and reports.
  • Provide crisis intervention and collaborative consultation with service providers.
  • Maintain participant files to ensure compliance and act as a peer review committee for file review.
  • Attend and conduct meetings and training as directed.
  • Support effective communication between case managers and specialists.
  • Establish and maintain positive relations with the public, funding agencies, participants, and other staff.
  • Ensure compliance with all organizational policies, standards, and safety regulations.

Requirements

What you’ll need
  • A bachelor's degree in social work or a related field is preferred.
  • Two years of successful experience in the delivery of human services or any combination of education, training and experience.
  • Knowledge and experience with social casework objectives, principles and methods.
  • Knowledge of Child Protective Services (CPS) and Adult Protective Services (APS).
  • Experience working with the homeless population and the barriers faced by the unhoused.
  • Extensive experience with strength-based case management and trauma-informed care models.
  • Experience working with low-income families from diverse social and ethnic backgrounds who have a history of homelessness or disability.
  • Ability to maintain a high degree of confidentiality and professional integrity.
  • Ability to demonstrate excellent administrative and organizational skills.
  • Ability to demonstrate excellent written and verbal communication skills.
  • Ability to demonstrate personal effectiveness through strong problem-solving and time-management skills.
  • Proven ability to represent the interests of the organization, interact effectively with a diverse population, and be comfortable building successful collaborative relationships with families and individuals, staff and other stakeholders.
  • Understanding of public welfare laws, principles of community organization, and effective use of public and private community resources.
  • Experience and skill to establish and maintain effective individual and group relationships with participant families, employees and community partners.
  • Proficiency working with computers including but not limited to Microsoft Office Suite and database systems.
  • Ability to react under pressure, use good judgment in ambiguous situations, and be flexible/adaptable.
  • Must pass all required background clearances in accordance with agency, County, State, and federal standards.
  • CPR/First Aid certification is required within 90 days of employment.
  • Completion and maintenance of all required training and health clearances.
