About the role
Key responsibilities & impact- Under direction of the Program Manager, responsible for the development of a case management plan to assist program participants
- Performs and provides a written assessment of program participant’s needs and develops individualized service plans
- Evaluates intensive short- and long-term individualized service plans on a regular basis
- Facilitates employment focused groups designed to meet participant needs
- Educates program participants on living skills, parenting, budgeting, health, hygiene, housing, and substance abuse
- Develops and maintains comprehensive lists of community resources for use by program participants
- Refers program participants accordingly and works closely with Employment and Housing Specialists
- Establishes individualized budgets with program participants
- Conducts in-home visits and provides necessary support services
- Documents all contact with program participants and prepares reports
- Maintains case notes and other documents in the Homelessness Management Information System (HMIS)
- Coordinates incoming and outgoing participants’ referrals with other community agencies
- Attends case conferencing, meetings and training as directed
- Performs other related duties as assigned
Requirements
What you’ll need- A Bachelor’s Degree in Social Work or a related field is preferred
- two years of successful experience in the delivery of human services
- Knowledge of social casework objectives, principles, and methods
- Knowledge of employment readiness, permanent/supportive housing, public welfare laws, and the functions of public welfare agencies
- Understanding of individual and group behavior and effects of social and economic forces upon individuals and families
- Principles and techniques of interviewing
- Computer skills/applications
- Principles of cultural competency and diversity
- Knowledge of challenges concerning dysfunctional families, domestic violence issues, problems unique to women, problems unique to families, disadvantaged and underserved populations, people of color, persons with disabilities, individuals with chemical issues, and the general homeless population
- Valid California driver's license and ability to meet the organization’s insurance carrier guidelines
- Must pass all required background clearances in accordance with agency, County, State, and federal standards
- CPR/First Aid certification is required within 90 days of employment.
