About the role

Key responsibilities & impact
  • Oversee therapy management in accordance with organization policies and protocols, identify and address medication errors and adverse reactions
  • Complete appropriate documentation promptly, following Client policies and procedures
  • Ensure adherence to established standards, client policies, and Visante procedures
  • Provide appropriate medication therapy and address patient concerns confidentially
  • Work with interdisciplinary teams to support effective patient care and program development

Requirements

What you’ll need
  • Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy
  • Current Pharmacist license as granted by the appropriate state licensing authority obtained within 2 months of hire date
  • American Heart Association BLS certification obtained within 6 months of hire date
  • 3 years of retail or specialty pharmacy preferred
  • Strong understanding of pathophysiology and pharmacotherapeutics
