About the role

Key responsibilities & impact

Provide direct service in a GLE/SIE residential setting
Help Persons Served overcome barriers and prepare for independent living
Implement Treatment Plan support interventions as assigned by the Team LPHA Clinician
Administer, document, store, and dispose of medications according to MAP policies, regulations, and company protocols
Teach Persons Served medication management
Provide crisis interventions and access on-call backup when needed
Collaborate with Integrated Team members to address Persons Served needs
Perform day-to-day assignments directed by the Program Director
Use recovery-oriented, person-centered, harm-reduction, trauma-informed, culturally competent, and strengths-based approaches
Maintain safety, physical site, Human Rights, licensing, and operational standards
Create opportunities for residents to participate in meal preparation and residence maintenance
Help Persons Served prepare meals
Encourage social interaction and foster a collaborative, welcoming residential environment
Advocate for Persons Served and serve as a resource for neighbors
Participate in trainings, supervision, and staff meetings
Assist with orienting new staff
Complete required documentation and incident reports
Contribute to quality improvement goals and objectives
Support evidence-based and best practices including Motivational Interviewing, Harm Reduction, Housing First, Stages of Change, WRAP, Seeking Safety, and Cognitive Behavioral interventions
Perform other related duties as assigned

Requirements

What you’ll need

High school diploma or equivalent required
Associate’s degree in a human service discipline preferred
One to two years of job-specific experience in the human services field preferred
Driving is required using either a Vinfen van or personal vehicle
Personal vehicle users must maintain adequate insurance and a safe driving record subject to annual checks
Valid US driver's license required at employment
Must be at least 21 years of age
Must have maintained a valid US driver's license for at least six months
Must pass a driver's screening background check
Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist Persons Served with daily living tasks
Ability to operate a computer, calculator, copier, and printer
Ability to communicate with Persons Served, staff, and external representatives
CPR required within two weeks of hire
First Aid required within two weeks of hire
MAP required within 150 days of hire
Strong commitment to Persons Served living, working, forming meaningful relationships, and receiving community resources and supports
Skills to establish supportive trusting relationships with Persons Served, families, and neighbors
Sensitivity to cultural, religious, ethnic, disability, and gender issues
Ability to balance competing priorities and make independent judgments and decisions
Ability to work professionally and confidentially
Knowledge of personal computer applications and equipment
