About the role
Key responsibilities & impact- Provide individual and group support to persons served in a residential program
- Teach skills that enhance independence, quality of life, and self-advocacy
- Maintain a safe, secure, clean, and well-maintained residential environment
- Support community integration, friendships, family engagement, employment, volunteering, and community participation
- Help individuals access community-based supports, resources, assistive technology, and adaptive equipment
- Plan recreational and social activities promoting inclusion and emotional well-being
- Provide training and support involving behavioral and rehabilitative interventions, fire and safety, appointments, money management, activities of daily living, shopping, cooking, chores, and generic resources
- Perform hands-on tasks and implement support plans
- Provide transportation as necessary
- Assess physical and emotional status and report changes to the supervisor
- Monitor individual funds and maintain records, progress notes, behavior and academic data, incident documentation, communication logs, membership logs, and activity schedules
- Procure, store, administer, document, and dispose of medication according to MAP requirements, regulations, and company policy
- Communicate with program nurses, case managers, Visiting Nurses Association, families, and other interested parties
- Participate in mandated training and staff meetings
- Ensure compliance with regulatory, corporate, divisional, safety, workers’ compensation, universal precautions, and exposure-control policies
- Provide shift coverage in other homes or programs as assigned
- Function as a human rights advocate and provide physical intervention or First Aid during emergencies
- Report to the Disabled Persons Protections Commission as required
- Assist in developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs
- Perform general housekeeping, snow shoveling, and trash removal
- Perform other related duties as required
Requirements
What you’ll need- One or more years of experience and/or training in a related function preferred
- High school diploma or equivalent required
- Experience may be substituted for academic training in some cases
- Valid driver's license required at employment
- At least 21 years of age
- Valid US driver's license held for at least six months
- Adequate insurance and safe driving record required when using a personal vehicle
- Must pass a driver's screening background check
- Ability to perform physically active work, including standing, walking, bending, kneeling, stooping, crouching, crawling, climbing, and navigating narrow spaces
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist persons served with activities of daily living
- Ability to remain stationary 50% of the time as needed
- Ability to speak, hear, and communicate with persons served, staff, and external representatives
- CPR certification required within two weeks of hire
- First Aid certification required within two weeks of hire
- MAP certification required within 150 days of hire
- PBS Tier 1 certification required within 150 days of hire
- Safety Care certification required within 90 days of hire
- Knowledge of advocacy, empowerment, self-advocacy, participatory planning, communication, household management, and crisis intervention techniques
- Ability to work independently and as part of an interdisciplinary team
- Ability to communicate effectively in oral and written formats
