  • Plan recreational and social activities promoting inclusion and emotional well-being
  • Support choice-making, decision-making, self-advocacy, and participation in support planning
  • Support access to assistive technology and adaptive equipment
  • Support work, volunteer, and community engagement opportunities
  • Provide training and support including behavioral and rehabilitative interventions, appointment scheduling, money management, ADLs, shopping, cooking, and household chores
  • Perform hands-on tasks and provide transportation as necessary
  • Assess physical and emotional status and report changes to the supervisor
  • Monitor individual funds
  • Maintain records, progress notes, behavior and academic data, communication logs, incident documentation, and activity schedules
  • Procure, store, administer, document, and dispose of medication according to MAP requirements, regulations, and company policy
  • Communicate with program nurses, case managers, Visiting Nurses Association, families, and other interested parties
  • Participate in mandated training and staff meetings
  • Ensure compliance with internal and external regulatory policies and standards
  • Perform housekeeping, snow shoveling, trash removal, and home maintenance
  • Provide shift coverage in other homes/programs as assigned
  • Function as a human rights advocate and provide physical intervention or First Aid during emergencies
  • Serve as a reporter to the Disabled Persons Protections Commission
  • Assist with developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs
  • Perform other related duties as required

Requirements

What you’ll need
  • One or more years of experience and/or training in a related function preferred
  • High school diploma or equivalent required
  • Valid driver's license required at employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Adequate insurance and safe driving record if using a personal vehicle
  • Pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist with activities of daily living
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to communicate effectively orally and in writing
  • CPR certification within two weeks of hire
  • First Aid certification within two weeks of hire
  • MAP certification within 150 days of hire
  • PBS Tier 1 certification within 150 days of hire
  • Safety Care certification within 90 days of hire
  • Knowledge of advocacy, communication, empowerment, self-advocacy, participatory planning, physical personal management, household management, and crisis intervention techniques
  • Ability to work independently and as part of an interdisciplinary team
  • Sensitivity to cultural, religious, racial, disability, and gender issues
