  • Teach skills that enhance independence, self-advocacy, quality of life, and community participation
  • Maintain a safe, secure, clean, and well-maintained residential environment
  • Help individuals access community-based supports and resources
  • Plan recreational and social activities promoting social inclusion and emotional well-being
  • Support individual choice-making, decision-making, friendships, family partnerships, employment, volunteering, and community engagement
  • Provide training and support involving rehabilitation, behavioral interventions, fire and safety, appointments, money management, activities of daily living, shopping, meal planning, cooking, and household chores
  • Support access to assistive technology and adaptive equipment
  • Perform hands-on tasks to implement support plans
  • Provide transportation as necessary
  • Assess physical and emotional status and report changes to the supervisor
  • Monitor individual funds
  • Maintain records, progress notes, behavior and academic data, incident documentation, communication logs, community membership logs, and activity schedules
  • Procure, store, administer, document, and dispose of medication in accordance with MAP requirements, regulations, and company policy
  • Communicate with program nurses, case managers, Visiting Nurses Association, and other interested parties
  • Participate in mandated training and staff meetings
  • Ensure compliance with internal and external regulatory policies, procedures, and standards
  • Complete housekeeping, snow shoveling, trash removal, and home safety and maintenance tasks
  • Provide shift coverage in other homes or programs as assigned
  • Function as a human rights advocate and provide physical intervention and/or First Aid during behavioral or medical emergencies
  • Serve as a reporter to the Disabled Persons Protections Commission
  • Assist in developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs
  • Perform other related duties as required

Requirements

What you’ll need
  • One or more years of experience and/or training in a related function preferred
  • High school diploma or equivalent required
  • Valid US driver's license required at employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least one year
  • Adequate insurance if using a personal vehicle
  • Safe driving record and ability to pass a driver's screening background check
  • CPR certification required within two weeks of hire
  • First Aid certification required within two weeks of hire
  • MAP certification required within 150 days of hire
  • PBS Tier 1 certification required within 150 days of hire
  • Safety Care certification required within 90 days of hire
  • May be asked to obtain CBIS certification
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, climb stairs, and maneuver through narrow spaces
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to assist clients with activities of daily living
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Required vision abilities including close, distance, color, peripheral, and depth perception, and ability to adjust focus
  • Knowledge and use of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, physical personal management, and household management techniques
  • Sensitivity to cultural, religious, racial, disability, and gender issues
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to communicate effectively in oral and written formats
  • Ability to work in a professional and confidential capacity
