About the role
Key responsibilities & impact- Initiate and conduct intake and engagement processes for assigned Persons Served within 72 hours
- Assess treatment and support needs and determine needs for Medicaid Rehab Option rehabilitative services
- Develop, sign and review Medicaid Rehab Option billable Treatment Plans with Persons Served, Peer Specialists and assigned staff
- Conduct Treatment Plan meetings, involve relevant parties, and plan for discharge
- Lead intervention teams comprising Outreach Workers, Nurses, Housing Coordinators, Addictions Recovery Counselors, Peer Specialists and other specialists
- Oversee service delivery, treatment and support skills planning, recovery goals, peer and family support, discharge readiness and transitions
- Deliver individualized, life-stage-appropriate services and supports
- Collaborate with team members to provide recovery, community, addiction, health, family and other services
- Promote access to healthcare and employment services, safe housing, community tenure and self-sufficiency
- Assign and direct Outreach Workers and train/direct GLE/SIE staff in delivering Treatment Plan interventions
- Perform, train, supervise, mentor and model evidence-based clinical and recovery-oriented interventions
- Ensure integration of team and GLE/SIE staff efforts and thorough documentation
- Support competency-based training and trauma-informed, person-centered, culturally competent and strengths-based practices
- Provide outreach to disengaged Persons Served and assist with care and life-stage transitions
- Develop and update Safety Plans and Crisis Prevention and Response Plans
- Interface with Emergency Services, employment coordinators and care coordination entities
- Conduct face-to-face visits within 48 hours after specified acute-care or community-return events
- Visit each assigned GLE/SIE twice weekly to supervise staff and check progress
- Provide field supervision to Outreach Workers and receive clinical supervision from the Assistant Team Leader
- Provide on-call clinical coverage as scheduled
- Participate in the LPHA/Clinician Community of Practice Workgroup
- Cover Assistant Team Leader duties during absences as needed
Requirements
What you’ll need- Master’s degree required in Social Work, Psychology, Counseling Psychology or other LCSW, LMHC or LPHA licensure-eligible program
- LCSW, LICSW, LMHC, LRC, LADC 1, OTR, RN or Licensed Psychologist in state of service required; eligible pathways may be approved by DMH
- MSW pathway candidates must obtain LCSW within 12 months of hire
- LMHC pathway candidates must pass their exam within 12 months of hire
- Clinical experience with people with psychiatric disability and/or co-occurring disorders preferred
- Access to an operational and insured vehicle and willingness to use it to transport Persons Served
- Valid US driver’s license; at least 21 years of age; license held for at least six months; safe driving record and ability to pass a driver screening background check
- CPR required within first two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- Ability to perform physically active duties, including standing, walking, bending, kneeling, stooping, crouching, crawling, climbing and lifting at least 25 pounds
- Ability to operate computers and office equipment
- Extensive knowledge of documentation standards and requirements
- Knowledge of clinical treatment principles, evidence-based practices, engagement strategies, crisis prevention and intervention, risk management, recovery-oriented and trauma-informed services
- Ability to formulate effective Treatment Plans, collaborate with team members, form supportive relationships, make independent judgments, and work professionally and confidentially
