About the role
Key responsibilities & impact- Assist in developing, implementing, and documenting individual treatment support plans
- Perform daily tasks with persons served, including rehabilitative interventions, behavioral interventions and monitoring, scheduling appointments, monitoring progress, and providing transportation
- Support persons served in becoming integrated into their communities
- Assess physical and emotional status and report changes to the supervisor
- Ensure program site safety and maintenance
- Participate in daily living and cleaning activities, including cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal
- Comply with corporate and divisional policies and safe work practices
- Store, administer, document, and dispose of medication according to MAP/MAC requirements and company policy
- Complete paperwork and logs
- Perform crisis intervention as needed
- Maintain current contact information with the Relief Pool Coordinator or Human Resources
- Perform other related duties as required
Requirements
What you’ll need- One to three months of experience in a human services setting
- In some cases, experience may be substituted for academic training
- High school diploma or equivalent required
- Valid US driver’s license may be required for driving duties
- At least 21 years of age if driving
- Valid US driver’s license maintained for at least one year if driving
- Ability to pass a driver screening background check if driving
- Safe driving record if driving
- Adequate insurance if using a personal vehicle
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist clients with daily living tasks
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- First Aid required within two weeks of hire
- CPR required within two weeks of hire
- MAC required, following Connecticut guidelines
- Safety Care required within 60 days of hire
